The R20 method is a protocol variation in which multiple passes of the laser are performed within a single session, separated by 20-minute intervals — enough time for the immediate tissue whitening response (frosting) from the previous pass to clear, allowing the next pass to penetrate more effectively.
Why the R20 Method Might Work
The theoretical basis for R20 is that the immediate whitening (frosting) produced by a laser pass is caused by gas microbubbles in the dermis created by rapid vaporization. These bubbles scatter and reflect subsequent laser pulses, reducing the energy reaching remaining ink particles. If the practitioner waits for the frosting to resolve — typically 20 minutes — the bubbles have dissipated and the next pass encounters the tissue in a more optically accessible state.
By performing 2 to 4 passes in a single session with 20-minute gaps, the R20 method attempts to achieve more ink fragmentation per clinic visit, potentially reducing the total number of sessions needed over the treatment course. This would be meaningful for patients whose primary constraint is time rather than cost.
The Clinical Evidence
Multiple published studies have examined the R20 method with generally positive results. A prospective study comparing single-pass and R20 four-pass protocols found significantly greater clearance in the R20 group, measured by objective colorimetric assessment at each timepoint. For patients motivated to reduce total treatment time and who have appropriate skin type and tattoo characteristics, R20 is a legitimate evidence-based option worth discussing with a knowledgeable practitioner. It is not a shortcut or a guarantee of faster results — it's a protocol variation with a reasonable evidence base for specific applications.
